INITIAL VISIT
In order for Dr. McCann to best treat your condition or injury, we need to have a comprehensive understanding of your current health status. When you come in for your initial consult, please bring all of the items listed below.

NEW PATIENT FORMS
The initial registration process for a new patient can be tedious and often requires information that you may have given to other medical offices in the past. We regret this inconvenience. You may expedite your initial visit registration process by printing the relevant forms listed below, completing them prior to your visit, and simply submitting them to the receptionist upon arrival for your appointment. Current demographics and insurance verification will be confirmed by our receptionist.
Patient Intake and History Form
Worker’s Compensation Report Information (if applicable)
No Fault Insurance Report Information (if applicable)
Notice of Privacy Practices (no need to print, simply review)
Once registration is complete, you will be escorted to an exam room by our Medical Assistant who will confirm the information you provided on the Patient Intake and History Form. Dr. McCann and his Physician Assistant will then perform a history and physical exam. Following his exam, Dr. McCann may order additional radiographs if required to establish a diagnosis and recommend a treatment plan.
After Dr. McCann’s consultation, you will meet with his office manager to schedule surgery if recommended or simply check out with the staff at the front desk to make a follow up appointment, if necessary, and receive prescriptions for any additional MRI testing or physical therapy that Dr. McCann may have recommended.
