Office Policies

The office is open Monday through Friday between the hours of 8:00 am- 5:00pm. The answering service will take calls before 9:00 am and after 4:00 pm.

Scheduling Appointments

New Patients

To schedule an appointment, please contact a doctor’s office listed. New patients will be asked to complete a standard new patient registration form and medical history form, both of which can be reviewed under patient forms.

Follow-up Appointments

If you are a non-surgical patient, and the doctor requests that you return for a follow-up visit, please be sure to make your return appointment prior to leaving our office or within a week following your original appointment. This will ensure your appointment is at a time convenient for you.

Insurance & Authorization

At least one of our surgeons participates in nearly all the insurance plans offered in the New York metropolitan area. Many of our surgeons are in-network and others are out-of-network providers. Please contact the individual office to confirm that they accept your insurance.

If you do not have the required insurance information at the time of your office visit, even if you have valid medical insurance, you will be asked to pay in full for your office visit if you wish to see the doctor. You will be reimbursed if you can later provide valid insurance information and/or referrals if necessary. If you do not wish to pay at that time, you always have the option to reschedule your visit.

Referrals and Pre-Authorization Requirements

Office Visits

Please be advised that any required insurance authorizations are the patient’s responsibility. Please obtain them before calling to make your appointment at the Orthopedic and Sports Medicine Department, if possible. They must be brought with you to your appointment or you may not be able to see the physician.

X-ray, MRI or CT-Scan Pre-Certifications

You must give our office at least 48 hours to obtain your pre-certification number. Once our office gets the pre-certification number, we will call you with that number and then you may schedule your appointment for your X-ray, MRI or CT-Scan. Medicare does not require any pre-certification.

Payment Method

Payments required on the day of your office visit may be made by cash, check, or credit card.

Billing Questions

Any questions regarding your medical bills should be directed to the office manager in your respective surgeon’s office.

Payments and Deductibles

All payments for office visits, if required by your medical insurance company, are due on the day of your appointment with the doctor. Any co-payments required by your insurance plan will be expected to be paid on the day of service, and for patients seeing our doctors out-of-network, the full office visit fee is payable at the time of the appointment.

Beth Israel Orthopedics and Sports Medicine will submit the routine bills for office visits directly to your insurance company for all in-network patients and will also extend the courtesy to submit bills to the insurance company for those patients who see our surgeons out-of-network. It is the office policy that the patient will fulfill the obligation of any deductibles required by their insurance provider.

Patient Requests

We will need 3-7 days to be able to process your request for x-ray, MRI, or CT Scans. Films are sent via regular mail unless you provide us with a credit card or a Federal Express account number. Please contact your physician’s office and press 2 for the office manager.

We will need a signed authorization from the patient for release of medical records.
